Registrations

Register Your Health Facility

To ensure quality healthcare delivery and uphold health standards in Edo State, all health facilities must be officially registered. Below you’ll find the required documents for registration.

  • Certificate of Full Registration
  • Certificate of Annual Practicing License
  • Degree Certificates and Other Relevant Staff Credentials
  • Two (2) Passport Photographs for Each Staff Member
  • Newspaper Publication of Name Change (for married females, if applicable)
  • Tax Clearance Certificate (3 years prior or Tax Identification Number Certificate)
  • Four (4) Copies of the Facility’s Building Layout
  • Four (4) Copies of the Facility’s Building Plan
  • Comprehensive Equipment List
  • Directions to the Facility (four copies)
  • Certificate of Incorporation (CAC)
  • Letter of Commitment: Pledge to share monthly patient attendance, morbidity, mortality statistics, and other relevant data with the Ministry of Health
